This story involves a grandmother who stays with her three grandchildren, she was neglecting herself by not taking her antihypertensive medication, which led to her having repetitive seizures because of her uncontrolled high blood pressure condition. She would frequently go to hospital for treatment and the grandchildren would be neglected.

Care For You Health Services Home Based Care brought this case under the attention of Department of Social Development Social workers which resulted in an intervention where they engaged in the care and arrangements of the grandchildren, and the children were taken to Frankfort by their uncle to attend school there.

The grandmother was discharged from hospital, and the social workers arranged for her to go to an old age home for rehabilitation as she had difficulty walking due to the stroke she had. Care For You Health Services has lent her a walker. There has been improvement for she can now walk without using a walker, the grandmother’s health has improved and her health condition and the treatment there of is controlled.
